The Role: managing a community-based caseload, primarily supporting Aged Care clients (small proportion of NDIS participants)

Your responsibilities will include:
• Functional assessments within clients’ homes
• Assistive technology prescription and equipment recommendations
• Minor and major home modification assessments
• Developing intervention plans to maximise independence and safety at home
• Reviewing client progress and completing high-quality clinical reports
• Working collaboratively within a multidisciplinary team
You’ll need:
• Current AHPRA registration as an Occupational Therapist
• Current driver’s licence and access to a vehicle
• Strong communication and time management skills
• A passion for delivering person-centred care and achieving great outcomes for clients
